Who Should I Refer?
Urgent Referral Criteria
Heart failure symptoms
NT-proBNP >2000 pg/mL
Within 2 weeks
Within 3 working days
Routine Referral Criteria
Previous diagnosis of Heart Failure on Chronic Disease Management Programme
Symptoms/signs suggestive of Heart Failure and NT-proBNP >400 pg/mL
Recent diagnosis of Atrial Fibrillation
Type 2 Diabetes with NT- proBNP >400 pg/mL
Asymptomatic murmur or family history of bicuspid aortic valve
Within 6 weeks
Within 10 working days
